Once enrollment is complete and tuition is arranged, your child is ready to begin their personalized learning journey at PTSEC. Welcome to the family!
Enrollment Requirements
To complete enrollment, please gather the following documents. Don't worry if you're missing something — contact us and we'll help you figure it out.
- Completed Enrollment Application(Provided by PTSEC)
- Most Recent Report Card(Last two grading periods)
- Standardized Test Scores(If available)
- Immunization Records (Florida Form 680)(Required for all students)
- Proof of Residency(Utility bill or lease agreement)
- Birth Certificate(Copy required)
- IEP or 504 Plan(If applicable)
- Withdrawal Form from Previous School(If transferring mid-year)
